Health Worker

Under the supervision of the Home Care Department, the Health Worker provides direct services to frail elders in their homes.
Responsibilities :
Provide personal care and assistance in clients' residences to help older adults live safely and comfortably at home. Tasks include homemaker services, personal care (e.g., bathing, peri-care, mouth, skin, and hair care), assistance with eating, toileting (including bedpan use), ambulation, laundry, shopping, glucose testing, blood pressure checks, and specimen collection.
Escort and assist clients to and from medical appointments.
Offer behavioral and emotional support, such as reality orientation and reminders for medical/nursing care and diet restrictions.
Assist with meal preparation, including special diets, and set up for meals at centers or for take-home.
Assist with special programs or participant needs under supervision, such as recreation, rehab exercises, or program committees.
Maintain cleanliness and safety of care areas, whether in homes or at centers.
Report participant status and progress regularly to home care staff or schedulers.
Complete administrative tasks, including reporting, record-keeping, and attending meetings.
Participate in training and orientation of new HWs and volunteers.
Promote positive public relations with participants, families, visitors, and the community.
Work a flexible schedule to provide services seven days a week, including some weekends.
Provide services across all districts of San Francisco.
Qualifications :
At least one year of experience working with older adults, paid or unpaid.
Desired Skills :
Certification as a Certified Nursing Assistant (preferred).
Language skills in Spanish, Russian, Vietnamese, Korean, or Chinese.
Sincere interest in working with frail elders.
Ability to work collaboratively in an interdisciplinary team, demonstrating initiative, resourcefulness, and flexibility.
Knowledge of the local community.
Willingness to work in a multilingual, multicultural environment.
Ability to understand complex written and verbal instructions.
Clear communication skills in English.
Physical Requirements :
Ability to lift, transfer, push/pull, maneuver, and reposition up to 35 lbs.
Ability to reach, bend, and walk.
Five-finger dexterity.
Compensation :
Range: $20-23/hr
This compensation range is indicative and may vary based on experience, education, and client needs. It does not include additional benefits or non-monetary perks. Institute on Aging reserves the right to modify job descriptions and work hours as needed.
