Kaiser Permanente Job Summary: As the leader of the health care team, provides professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. Demonstrates performance consistent with the Medical Center Vision, Mission, and Strategic Plan of the organization. Remains flexible to changing systems; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats co-workers, patients, families, and all members of the health care team with dignity and respect.

Essential Responsibilities

Leadership

Upholds Kaiser Permanente’s Policies and Procedures, Principles of Responsibilities, and applicable state, federal and local laws.

Leads the health care team; delegates tasks appropriately, and demonstrates accountability.

Understands own and team members’ scope of practice and escalates issues as appropriate.

Demonstrates professional, supportive behavior. Champions new ideas and leads others through change.

Uses effective communication strategies, including chain of command and issue escalation, to achieve outcomes.

Participates in problem identification and resolution. Mentors, orients, and coaches others in unit operations and patient care activities.

Shares responsibility and authority with subordinates and holds them accountable for performance.

Collaborates with other departments to resolve member problems. Prioritizes, delegates, and supervises work to ensure completion of patient care activities.

Complies with regulatory requirements, policies, procedures, and standards of practice.

Nursing Process

Develops and contributes to the individualized plan of care reflecting assessment, planning, implementing, and evaluating outcomes.

Ensures multidisciplinary planning, consultation, and education are included in the plan.

Ensures the plan reflects admission or outpatient data, age-appropriate care, cultural needs, and appropriate acuity.

Discusses the plan with patient, family/significant others, and completes in a timely manner.

Monitors patient progress and revises the plan as needed based on condition and progress.

Ensures care meets standards of practice and supports discharge planning and patient education.

Ensures patient safety, including medications and procedures, falls prevention, decubitus prevention, and infection prevention.

Ensures optimal pain control and communicates patient anxieties or concerns regarding condition, treatment, or discharge. Ensures understanding of medication purpose, side effects, and instructions at discharge.

Documentation

Charting is accurate, legible, dated, and timed.

Documentation reflects the nursing process and interventions and evaluations.

Utilizes computer systems effectively for patient care.

Clinical Outcomes

Discusses patient findings and progress toward outcomes with physicians and the health care team.

Demonstrates competencies during probation and ongoing departmental validation.

Makes comprehensive nursing decisions based on data interpretation, assessments, and outcome evaluations.

Participates in departmental performance improvement activities (planning, measuring/monitoring, assessing, and improving).

Workplace Safety

Adheres to safety principles and practices; applies standard precautions and maintains a safe environment for self and others.

Patient Care Experience

Practices customer service standards as defined by the Service Area, Medical Center, and department.

Responds promptly to call lights, alarms, and patient requests; protects patient confidentiality; maintains a clean and functional work environment.

Treats patients and families with courtesy, respect, kindness, and compassion; provides information in an understandable way and ensures comprehension.

Delivers a patient care experience that meets or exceeds expectations.

Team Commitment

Works effectively as a flexible, cooperative team member; addresses conflicts constructively and seeks appropriate assistance.

Accepts accountability and provides mentorship to new staff, students, and other team members.

Participates in mandatory meetings and department professional development; contributes to unit learning needs and annual education planning.

Maintains attendance and supports a collaborative Labor-Management partnership; stays informed of unit activities and recommends changes.

Fiscal Responsibility

Organizes work to minimize overtime and identifies systems improvements to simplify processes.

Uses payroll and non-payroll resources efficiently.

Experience

Minimum six (6) months of experience required in an acute or sub-acute facility within the last three (3) years.

Education

N/A

License, Certification, Registration

Registered Nurse License (California)

Basic Life Support from American Heart Association

Additional Requirements

Basic arrhythmia required.

Notes

Will be required to work every other weekend.
