Work schedule:
Minimum of 40 hours per week with at least 36 hours of patient contact. Days vary based on clinic hours.
Call expectations:
Participate in shared clinic call rotation, including evenings, weekends, and holidays. A Family Care physician provides backup support.
Department overview:
Part of a team providing comprehensive, evidence-based care in a Medical Home environment, supported by Behavioral Health, Pharmacy, Care Managers, and other staff. Responsibilities
Deliver high-quality family medicine care, aligning treatment with patient values. Collaborate with team members and referring providers to ensure excellent patient experiences. Provide compassionate, patient-centered care through active listening and effective communication. Prescribe medications following policies, consulting with pharmacists and psychiatrists as needed. Accurately document all care in the EHR, including timely reporting of diagnostics. Educate patients and families on health issues and treatments. Participate in after-hours call duties and quality improvement initiatives. Consult with physicians and have chart notes reviewed or approved as required. Qualifications
Education
Required: Master of Science in Physician Assistant Studies or Nursing. Preferred: Doctorate in Physician Assistant Studies or Nursing Practice. Licensure/Certification
Unrestricted PA or Nurse Practitioner license; NCCPA or AANC/ANNP certification. DEA registration, BLS, and ACLS if practicing sedation. Experience
Outpatient family medicine experience and EHR proficiency. Preferred: 2+ years in family practice, familiarity with Medical Home model, and Epic EHR experience. Additional Details
