is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. Form Health provides high-quality expert care and leverages technology to enhance the patient experience. All Form Health patients work closely with their care team, which includes board certified physicians, advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. About the Role We are hiring experienced Nurse Practitioners who are passionate about and who have recent experience in obesity medicine to provide longitudinal telehealth care for patients with obesity. In this role, you would collaborate with physicians and registered dietitians to deliver exceptional team-based care. This will be a full-time, remote position.
Qualifications We seek applicants who:
Have a Nurse Practitioner degree, with experience practicing Obesity Medicine, including effective evidence-based lifestyle counseling and pharmacotherapy
Are licensed to practice and prescribe legally, and credentialed with major insurance payers, in the state of California
Have 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2 + years of obesity medicine experience in a primary care, internal medicine, or similar setting
Have excellent interpersonal skills for patient and team interactions
Are committed to the highest quality of medical care and an evidence-based approach to obesity management
Have prior experience conducting telemedicine visits and who have a great web-side manner
Bilingual candidates with medical fluency in Spanish are highly preferred
