Overview
Nurse Practitioner/Physician Assistant (Must be able to conduct home visits) Alignment Health is dedicated to serving seniors and those who need it most: the chronically ill and frail. The Advanced Practice Clinician in the Care Anywhere (CAW) program provides care and support to Alignment Healthcare patients in the comfort of their own homes, with goals to prevent unnecessary hospitalizations and manage disease progression through timely clinical interventions. Position type: Full-time Location: Orange, CA area (home visits)
Responsibilities
Conduct in-home assessments on Alignment members, perform medication reviews, and conduct health screenings. Provide patient education by aligning care desires with best care given. Identify diagnoses to be assessed in care management and active medical management. Partner and communicate with Regional Medical Officer and other team members to discuss and develop the most appropriate care plans based on member/patient needs. Coordinate care with multiple stakeholders, including PCPs, specialists, and ancillary providers. Lead broader clinical team which may include nurses, health coaches, social workers, and care coordinators.
Experience
Preferred: One (1) year of prior clinical or home care experience. Previous EMR experience preferred. Experience in care of older adult (geriatric) patients preferred.
Education
Required: Master’s degree from an accredited NP Program or PA program.
Specialized Skills
Ability to communicate positively, professionally and effectively with others. Problem-solving, organizational and time management skills; ability to work in a fast-paced environment.
Licensure
Required: Active Nurse Practitioner or Physician Assistant license. Other requirements: Active RN license and Furnishing number (Must, upon hire) for NP only; Active Nurse Practitioner Board Certification/Physician Assistant Certification; NPI Number, DEA, Valid BLS; Valid Driver License and current automobile insurance.
Essential Physical Functions
Regularly communicate via talk or hearing; stand, walk, sit, and manipulate objects; reach with hands and arms. Lift and/or move up to 20 pounds; close vision and ability to adjust focus.
Pay
Pay Range: $130,332.00 - $195,498.00 Pay range may be based on location, education, responsibilities, experience, etc.
