Full Time Physical Therapist Case Manager Anticipated Territory: Pacifica Area In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment. Your goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and to help the client reach his/her maximum performance within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs. You may supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Other responsibilities are included below.
What you’ll do as a Home Health Physical Therapist Case Manager
Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan.
Help patients with mobility, movement, and regaining independence.
Create detailed treatment and care plans for each patient.
Communicate and coordinate with patients’ physicians to ensure a quality level of care.
Evaluate and report patients’ progress to the physician.

To be successful as a Physical Therapist
Current unrestricted CA Physical Therapist License
Current BLS/CPR Certification
Valid CA Driver’s License
Knowledge of local community resources
Understanding of development, behavior, and cultural influences on health care
Knowledge of state and federal homecare regulatory guidelines (preferred)
Home health care experience (highly preferred)
Experience with high-acuity patients (a plus)
Experience working with an interdisciplinary team (highly desired)
Case management fundamentals (highly desired)
Self-directed, empathetic, critical thinker with outstanding interpersonal skills
Leadership and organizational abilities
$120,000 - $135,000 a year
Wage range details: Yearly compensation includes a base salary with potential bonuses. Starting pay is commensurate with relevant experience; 21HHS offers bonus incentive plans, health benefits, and 401(k) with up to 4% match. Typical progression may include PTI/PTII levels with corresponding pay scales and bonuses.
