Position: Case Manager - Community Living Services
Responsible for the assessment of resident psychosocial needs and coordination of support services for low-income older adults. Plans for and monitors services and interventions ensuring provision of quality care.
SPECIFIC RESPONSIBILITIES:
Conducts comprehensive assessments and ongoing re-assessments of residents, including psychosocial, physical, mental health, environmental, and spiritual needs. Writes comprehensive assessments and develops client-centered Community Living Plans based on assessment information. Identifies, arranges, and monitors appropriate community services, with knowledge of Medicare, Medi-Cal, and other entitlement programs. Establishes and maintains care management relationships with clients and their significant others, providing respect, dignity, support, crisis intervention, advocacy, problem solving, and therapeutic interventions. Meets with clients at least monthly, reviews and updates their Community Living Plans regularly. Documents all case management activities via progress notes within 24 hours, updating service plans as needed. Maintains accurate paperwork and follows a clear, consistent charting system for continuity of care. Educates and trains clients and their support systems on resources and self-management when possible. Maintains open communication with community providers, including physicians and social workers, ensuring confidentiality. Monitors the quality and quantity of services provided by other providers. Collaborates with clients, caregivers, and services to terminate clients when appropriate, documenting the process. Participates in research and data collection as required. Engages in ongoing quality improvement efforts. Attends team meetings and contributes to communication and problem-solving within the organization. Utilizes supervision effectively, providing updates on caseload activities. Incorporates NASW ethical standards into all interactions. Adheres to regulatory, procedural, and organizational policies. Attends continuing education to enhance skills and knowledge relevant to the role. Performs other related duties as assigned. Qualifications:
B.A. or B.S. in Social Work or related field required. Minimum one year’s experience with disabled adults and/or frail elderly. Knowledge of medical and psychosocial issues affecting this population. Bi-lingual in English plus Cantonese, Mandarin, and/or Spanish preferred. Strong communication and presentation skills. Case management experience in community health care. Detail-oriented with problem-solving skills and ability to prioritize. Computer literacy required. Physical & Language Requirements:
Ability to lift up to 25 lbs. Ability to reach, bend, walk. Five-finger manipulation. Clear spoken and written English. Compensation:
Range: $71,797 - $85,808 annually. This amount is not necessarily reflective of actual compensation and does not include additional benefits or non-monetary incentives. Important Notices:
