Responsibilities
Administrative Functions: Assist the Director of Nursing Services in directing the day to day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the long term care facility.
Participate in developing, maintaining, and updating written policies and procedures that govern the day to day functions of the nursing service department.
Ensure that reference material (i.e., PDRs, regulations, professional standards of practice, etc.) maintained at the nurses\' stations is current.
Recommend written material to assist the nursing service department in meeting the day to day needs of the resident.
Ensure that the Nursing Service Procedures Manual is current and reflects day to day nursing procedures performed in this facility.
Ensure all nursing service personnel comply with the procedures in the Nursing Service Procedures Manual.
Make written and oral reports/recommendations to the Director as necessary concerning the operation of the nursing service department.
Periodically review the department\'s policies, procedure manuals, job descriptions, etc., and make recommendations for revisions to the Director.
Assist in coordinating nursing services with other resident services to ensure continuity of residents\' total regimen of care.
Ensure that all nursing service personnel are in compliance with their respective job descriptions.
Participate in the development, maintenance and implementation of the facility\'s quality assurance program for the nursing service department.
Participate in facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services.
Periodically review the resident\'s written discharge plan and assist in updating it as required. Plan the nursing services portion of the resident\'s discharge plan as necessary.
Meet with the nursing staff and support personnel to plan the department\'s services, programs, and activities.
Admit, transfer, and discharge residents as required. Complete accident/incident reports as necessary. Interpret the department\'s policies and procedures to personnel, residents, visitors, and government agencies as required.
Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations to the Administrator. Agree not to disclose resident\'s protected health information and promptly report suspected or known violations to the Administrator.
Report any known or suspected unauthorized attempt to access facility\'s information system.
Committee Functions
Attend various committee meetings of the facility (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment) as required.
Assist in preparing written and/or oral reports of the nursing service programs and activities.
Provide information to the Quality Assurance and Assessment Committee as requested.
Participate in developing the agenda for the Care Plan Committee meetings as necessary.
Participate in regularly scheduled reviews of the discharge plans and make such plans available to the Care Plan Committee as required.
Implement recommendations from established committees as instructed by the Director.
Personnel Functions
Ensure a sufficient number of licensed practical and/or registered nurses are available for your tour of duty to maintain quality care.
Ensure a sufficient number of certified nursing assistants are available for your tour of duty to provide routine nursing care.
Develop work assignments and delegate administrative authority, responsibility, and accountability as needed.
Ensure all nursing assistants are enrolled in or have graduated from an approved nursing assistant training program; supervise nurse aide trainees under a licensed nurse.
Make daily rounds to ensure nursing personnel perform according to acceptable nursing standards; report findings to the Director.
Monitor absenteeism; report problem areas. Ensure policies and dress codes are followed. Review complaints and grievances. Report as required.
Develop and maintain a positive working rapport with interdepartmental staff to meet residents\' needs; foster a warm, calm environment.
Meet with shifts regularly to identify and correct problems and improve services. Review workforce and participate in performance evaluations and staffing decisions.
Ensure fair disciplinary action and comply with equal opportunity standards. Report occupational exposures and incidents as required.
Ensure unattended departmental computers are logged off or protected, following facility policy guidelines.
Nursing Care Functions
Inform nursing staff of new admissions, room assignments, and arrival times; prepare rooms and greet residents on admission.
Escort admissions to rooms as necessary; make rounds with physicians; encourage timely documentation of progress notes and orders.
Consult with residents, families, and physicians in planning care; notify changes in condition; observe residents daily and ensure licensed supervision for direct nursing care.
Review nurses\' notes for completeness; provide direct nursing care as needed. Manage IV start, specimen collection, vital signs, and admissions/discharges.
Monitor medication administration, review medication cards, and ensure compliance with policies. Inspect treatment areas daily.
Meet with residents/families as needed; assist in transportation for discharge; inform families of deaths and coordinate with funeral homes if requested.
Staff Development
Participate in developing, planning, and scheduling in-service training; assist in annual facility in-service programs (OSHA, TB, HIPAA, Abuse Prevention, Safety, Infection Control).
Plan clinical supervision for nurse aide trainees and attend continuing education to maintain licensure.
Safety and Sanitation
Assist in developing safety standards; ensure staff receive appropriate in-service training for procedures involving blood/body fluids.
Maintain PPE stock; enforce proper use, disposal, and hand hygiene; participate in drills; maintain infection control programs.
Develop procedures for reporting hazardous conditions; ensure use of safety equipment; verify MSDS availability; ensure clean, sanitary work areas and resident rooms.
Equipment and Supply
Recommend equipment and supply needs; maintain adequate stock; assist in preventive maintenance and safe operation of equipment.
Ensure trained personnel operate equipment and that supplies are used efficiently; maintain MSDSs for hazardous chemicals.
Care Plan and Assessment
Participate in assessments and development of care plans for residents; encourage resident participation; review and revise plans as needed.
Ensure staff are aware of care plans and refer to them when delivering daily care; assist in planning, scheduling, and revising MDS and RAPs.
Budget and Planning
Stay informed about economic conditions and recommend adjustments to maintain quality care; report suspected fraud.
Supervisory Requirements
Assist with supervision and management of the nursing staff; RN is delegated administrative authority, responsibility, and accountability for assigned duties.
Qualifications
Education and/or Experience: Nursing Degree from an accredited college or university; training in rehabilitative/restorative nursing; 2+ years in long-term care preferred.
Language: Read, write, speak, and understand English; ability to read technical procedures and policy manuals; effective communication with managers and staff.
Mathematical/Reasoning: Ability to apply fractions, percentages, ratios; independent decision-making; problem solving; teamwork and leadership.
Licenses/Certifications: Current, unencumbered RN license; CPR certification; PCC knowledge preferred; maintain continuing education/licensing; good standing with State Board.
Physical Demands: Ability to move intermittently; lift/move up to 25 pounds; meet health requirements; able to work with ill or emotionally upset individuals; capable of evacuations if necessary.
Work Environment
Works in office and nursing service areas; subject to interruptions, exposure to residents, visitors, and staff; may require extended hours and call-backs; exposure to infectious waste and chemicals; potential for injuries and environmental conditions.
