Part Time Physical Therapist Case Manager Coverage Area: San Francisco County Responsibilities
In partnership with other clinicians on the case, provide skilled services to evaluate and address the needs of individuals in their homes, enabling them to live safely and productively at home. Evaluate and treat patients using medically prescribed physical therapy programs; may supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Perform an evaluation in the home and develop a care plan; work with PTAs to follow the care plan. Help patients with mobility and independence; create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure high-quality care. Evaluate and report patients’ progress to the physician or medical director. Qualifications
Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver’s License Knowledge of local community resources; understanding of development, human behavior, and cultural influences on health care Knowledge of state and federal homecare regulatory guidelines (preferred) Experience in home health care (highly preferred); experience with high-acuity patients is a plus Experience working with an interdisciplinary team (highly desired); familiarity with case management fundamentals (highly desired) Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills Organized and able to manage responsibilities effectively Salary and benefits $120,000 - $135,000 per year, with potential bonuses. Starting pay is commensurate with relevant experience. Benefit details include health, dental, life insurance, 401(k) with matching, PTO, and more.
