Job Summary The Psychiatric Nurse Practitioner will provide general outpatient psychiatric services to members of our onsite community, including psychiatric intakes and evaluations for acute and chronic conditions along with related treatments, medication evaluations and treatment option planning. This role will work closely with our primary and ancillary care teams, including other behavioral health providers, as part of our larger comprehensive primary health model. Job Responsibilities

Assess and treat acute and chronic psychiatric conditions through a combination of psychotherapy, psychoanalysis and medication management; discuss progress toward goals with patients, provide instructions, monitor effect of medication(s), with referral for inpatient services as needed. Develop individualized care and treatment plans by determining the nature and extent of cognitive, emotional, developmental, social, and behavioral disorders, and establishing treatment goals and methodologies, collaborating with behavioral health providers as appropriate. Engage in regular discussions with patients to change behavioral patterns, explore past experiences, and/or conduct group and family therapy sessions. Provide treatment for acute and chronic psychiatric conditions. Complete medication reviews and prescribe medications. Advise or inform guardians, relatives, or significant others of patients' conditions or treatment, as appropriate. Provide evidence-based, timely, compassionate care. Work with a multidisciplinary medical team, including primary care providers, health coaches and other behavioral health specialists. Complete thorough documentation using EMR and other integrated tools and applications, collecting medical information from patients, family members, and other medical professionals. Actively participate in quality improvement programs, as well as in multidisciplinary quality assurance programs. Collaborate with the management team as needed to ensure improvement of patient care. Cooperate with clinical outcome reviews and measurements. Comply with all Crossover Health policies, procedures, and protocols. Performs other duties as assigned. Required Qualifications

NP degree from an accredited program. Active Washington State Medical License. Completion of accredited Psychiatric-Mental Health Nurse Practitioner (PMHNP) Post-Master’s certificate program. American Nurses Credentialing Center (ANCC) Psychiatric-Mental Health Nurse Practitioner (PMHNP) certification. Minimum 5 years of clinical experience serving patients across the care continuum of psychiatric care. Current BLS (Basic Life Support) certification. Preferred Qualifications

Experience in an outpatient setting, specifically within an integrated primary care model. Excellent communication skills both verbal and written, including presentation and consultation skills. Experience with telehealth or virtual care offerings, including aptitude for associated technology. Competent using computer applications including database, spreadsheet, and document creation. Self-directed individual with polished professionalism and the ability to prioritize multiple tasks. Ability to use discretion and judgment in dealing with sensitive or confidential information. Passion for best-in-class member experiences (patient-centered medical home model) and for providing next generation health care. Thrives in a fast-paced and rapidly changing, demanding environment. Experience using EMR systems. Physical Job Requirements

May require standing, walking and sitting for extended amounts of time. Occasionally lift and carry items weighing up to 50 lbs. Manual and finger dexterity and hand-eye coordination. Includes full range of body motion including potential of handling and lifting patients. Requires corrected vision, hearing and speech within normal ranges. Must be able to effectively communicate with patients and team members.
