Family Physician

What You’ll Do
Review patient records including allergies, medications, problems, and immunization status. Obtain and document complete medical, social, and family histories. Perform comprehensive physical exams and routine preventive care for children, adolescents, adults, and seniors. Order, interpret, and analyze diagnostic tests and imaging to support clinical decision-making. Diagnose conditions and prescribe appropriate medications, therapies, and treatments. Administer and prescribe vaccinations across the lifespan. Provide prenatal and postpartum care within scope of training and organizational practice model. Counsel patients and families on preventive health, lifestyle, mental health, hygiene, and disease prevention. Perform minor office-based procedures consistent with training and competency. Refer patients to specialists or behavioral health providers when indicated. Document all encounters, diagnoses, treatment plans, and follow-up instructions in the EHR. Monitor and follow up with high-risk or urgent patients to ensure continuity of care. Participate in Quality Assurance/Quality Improvement (QA/QI) activities and clinical team meetings. Collaborate with integrated care teams (medical, dental, behavioral health, and enabling services). Represent the health center at community events and outreach activities as appropriate. Engage in ongoing training in cultural competency, evidence-based practice, population health, OSHA, HIPAA, and EHR use. Mentor and support team members to foster a positive, collaborative work environment. Adhere to organizational policies, procedures, and code of conduct. Perform other duties as assigned.
What We’re Looking For
M.D. or D.O. degree with completion of an accredited Family Medicine residency. Current license to practice medicine in the State of Wisconsin. Board Certification by the American Board of Family Medicine (ABFM) or American Osteopathic Board of Family Physicians (AOBFP). DEA registration and Basic Life Support (BLS) certification. 2–3 years of clinical experience preferred; prior work in community health or underserved populations strongly preferred. 2–3 years of experience in people management including coaching, mentoring, and team development. Demonstrated ability to work effectively in a team-based, integrated care environment. Strong interpersonal and communication skills with commitment to patient-centered care. High ethical standards and respect for confidentiality. Bilingual (Spanish) strongly preferred.
