Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals. No one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs. The Family Physician is responsible for providing quality primary health care to patients while working closely with other healthcare staff in a team approach. This person is a credentialed, licensed physician who is a salaried (exempt) employee providing primary care medical services. Trauma-informed de-escalation skills preferred but not required. Excellent interpersonal and customer service skills required. Special Skills/Training:
Strong verbal and written communication skills. Bilingual in English/Spanish/Dari/Punjabi/Vietnamese preferred. Must have strong time management skills, ability to prioritize, and work in a fast-paced environment. Knowledge of Electronic Health Record and associated computer programs. Ability to collaborate with mental health and substance use treatment providers and work well with the primary care team. Demonstrated interest and willingness to teach and mentor residents and medical students. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Provides assessment/evaluation, diagnosis, treatment/prescribing, planning, and education for patients. Provides medical services including acute care, re-evaluations, and management of chronic conditions, possibly in specialized areas. Reviews and manages patient information, labs, diagnostic images, prescriptions, outside consultation notes, medical records, referrals, etc. Prescribes and administers medications according to assessed conditions, reviewing current medications to prevent adverse reactions. Refers patients to specialists and relevant services as appropriate. Prioritizes patient care throughout the day. Provides acute triage over the phone or in person. Educates patients on diseases, providing handouts as appropriate. Performs in-office procedures as directed. Documents all care in the electronic health record system. Responds to messages and relays information to patients and external sources. Completes medical forms as needed. Coordinates patient care activities of nursing and support staff. Participates in quality assurance, care team development, and system optimization. Assists with outreach programs. Participates in education of students. Responds to after-hours triage calls on a rotation basis. Maintains hospital privileges, licensure, CPR, and DEA certification. Shares on-call and hospital rounding responsibilities, including billing for off-site encounters. Functions as a member of the medical team, maintaining communication and attending meetings. Participates in quality improvement, peer review, and administrative functions. Maintains ongoing clinical competence through CME activities. Maintains professional and ethical standards.
