About Greenbrook Medical: we provide high-touch, relationship-based primary care to seniors, built around the idea of delivering the care we’d want for our own parents. We focus on patient outcomes in a value-based framework and support patients through their healthcare journey with a dedicated care team. About the Role

Primary Care Physicians manage daily care for patient panels capped at approximately 500 patients. The model emphasizes work-life balance and a concierge-like patient experience. Typical daily patient visits are 30 to 45 minutes, with around 12 patient visits per full day. Clinic hours are generally 9:00 a.m. to 3:30 p.m., plus time for urgent needs and administrative tasks. A dedicated team of coders handles pre-charting and post-visit reviews, and medical assistants support administrative tasks and care coordination. Locations for new clinics and expansion clinics include: Spring Hill, FL (spring to fall 2026) Sun City Center, FL (fall 2026) Potential additional Greater Tampa Bay Area locations Office Hours: Monday–Friday 8 a.m.–5 p.m. Key Responsibilities

Provide comprehensive, patient-centered care for geriatric patients within a managed, value-based care model. Deliver care that aligns with evidence-based guidelines to optimize outcomes and quality metrics. Develop and implement personalized care plans; manage chronic conditions, preventive care, and acute issues within value-based care principles. Coordinate care with the healthcare team, including specialists, and participate in team meetings to review patient progress. Utilize electronic medical records (Epic or eCW) to document care, track outcomes, and support care coordination. Monitor patient progress, address care gaps, and conduct routine follow-ups to ensure adherence to care plans. Educate patients and caregivers on disease management, preventive care, and healthy aging. Accountabilities

Build and maintain a thriving patient panel of ~500 individuals in collaboration with growth/onboarding teams. Identify and address care gaps to ensure comprehensive and preventive care; aim to reduce unnecessary hospitalizations through effective outpatient management. Strive for high Patient Net Promoter Scores (NPS) by fostering trust and satisfaction in every interaction. About You

Experience: 2–6 years in a primary care setting (preferred managed care) or a recent residency graduate. Licensure: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar; Florida license or eligibility; board-certified or board-eligible (preferred; board eligibility required). Skills

Market-leading compensation with three components: base salary, qualitative performance bonus, and clinic performance bonus (starting in year three). First two years compensation may approach $300k; potential to exceed $400k with bonuses. Paid time off, monthly benefits stipend, and 401(k) with company match. Our Selection Process
