Position : Full Time Physical Therapist Case Manager Anticipated Territory : San Mateo County, from San Bruno to South San Francisco Responsibilities
In partnership with other clinicians on the case, provide skilled services to evaluate and address the needs of individuals in their homes to help them live safely and productively at home. Evaluate and treat patients using medically prescribed physical therapy programs; restore function to prevent or minimize disability. Supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards where applicable. Perform home evaluations, develop care plans, and work with PTAs to follow the plan. Help patients with mobility and independence; create detailed treatment and care plans. Communicate and coordinate with patients’ physicians to ensure quality care; evaluate and report progress to the physician as needed. Why work with us

Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver's License Knowledge of local community resources Understanding of development, human behavior, and the influence of culture on health care Knowledge of state and federal homecare regulatory guidelines (preferred) Home health care experience highly preferred Experience with high-acuity patients is a plus Experience working with an interdisciplinary team is desirable Case Management fundamentals are desirable Self-directed, empathetic, critical thinker with outstanding interpersonal skills Leadership, organizational and planning abilities Compensation
$120,000 - $135,000 per year. Wage details vary by experience and role. 21st Century offers a comprehensive benefits package, including annual bonuses and potential for advancement to higher salary tiers.
