Job Description

Summary:

Under general supervision, diagnose and treat patients in collaboration with physician colleagues. Provide continuity of patient care by coordinating communication between the health care team, patient and family. Coordinate care to ensure quality, cost-effective care; appropriate length of stay; and patient safety. Serves as a link integrating relevant research and best practices.

This position is open to Family or Acute Care Nurse Practitioners and Physician Assistants.

This position will be primarily outpatient with some inpatient work. The role is a M-F schedule, 8am-4pm.

Principal Responsibilities

The following statements describe the general nature and level of work performed by the provider in this classification. This is not an exhaustive list of all job responsibilities. A detailed description specific to performance functions will be described in the Nurse Practitioner’s collaborative written agreement.

Clinical Responsibilities

Provide day-to-day organizational and clinical direction and leadership of the healthcare provider team in collaboration with the attending physician.

Perform comprehensive history and physical assessments for patients.

Order, obtain and interpret diagnostic tests.

Establish medical diagnoses based on history, assessment and diagnostic findings.

Design, order and document treatment plans including prescriptions based on review of HPI and results.

Initiate protocols/clinical pathways and evaluate the plan of care for timely intervention.

Perform invasive procedures as appropriate.

Collaborate with attendings and residents regarding admission, discharge and transfer decisions.

Present patients in rounds and communicate plan of care to providers, patients and families.

Provide patient and family instruction related to plan of care, disease process, treatment plans, and medications.

Counsel and educate patients and families on disease processes, health maintenance and preventive care.

Initiate referrals and specialty consultations as needed.

Facilitate patient flow and collaborate with the multidisciplinary team to ensure timely discharge.

Provide specialty consultations and advocate for patient participation in care.

Consider holistic needs and coordinate resources (e.g., pain service, palliative care).

Perform other activities as delineated in individual NP written agreements.

Education Responsibilities

Provide formal and informal educational programs for health care team members.

Mentor and precept students; promote understanding of disease processes.

Serve as a clinical resource for interns, residents, fellows, nurses and students.

Provide community education, health promotion and outreach.

Act as a community health resource for the specialty.

Clinical Leadership/Outcomes

Coordinate NP coverage and collaborate with physician leadership to meet service metrics (clinical outcomes, LOS, cost of care, patient satisfaction).

Participate in performance improvement and quality assurance activities.

Develop clinical pathways and promote evidence-based practice.

Influence hospital policy to improve care delivery and patient outcomes.

Introduce and evaluate new care delivery models responsive to patient needs.

Participate in department and organizational initiatives; maintain compliance and regulatory documentation.

Supervise ancillary staff and perform annual reviews when applicable.

Customer Service

Model a customer service focus for internal and external customers; maintain patient confidentiality.

Professional Development and Leadership

Serve on department, hospital and community committees; maintain licensure and certifications as required.

Participate in educational opportunities and professional organizations.

Pursue growth opportunities; promote the organization to customers.

Research Responsibilities

Clinical trial sub-investigator or principal investigator for nursing research as appropriate.

Qualifications

Master of Science degree with completion of a Nurse Practitioner program. Current Maryland NP certification and DEA/CDS eligibility as applicable.

CPR required; ACLS/BCLS as required for inpatient work.

Minimum one year of RN experience.

Demonstrated professionalism through involvement in professional activities, teaching, research, or certification in the specialty.

Knowledge of growth and development principles; ability to assess data relevant to population needs.

Strong verbal and written communication skills; effective interpersonal skills.

Maintains required safety training and updated immunizations.

Working Conditions

Weekend, shift work, holidays, on-call and overtime may be required.

Potential exposure to infectious diseases; follow Universal Precautions.

Patient Safety

Take action to correct risks to patient safety; report adverse events; implement policies and standards.

Maintain effective team communication and identify risks in processes and devices.

Pay Range: $54 - $77
