Job Description Summary

Under supervision and in collaboration with the physician staff, the Advanced Practice Provider (APP) performs physical examinations and treats common episodic and chronic healthcare problems with emphasis on health maintenance and disease prevention. This position is designed for healthcare professionals, such as nurse practitioners and physician assistants, who possess advanced training and expertise. Primary responsibilities include providing patient care in the clinic setting, performing in-office procedures, and focusing on patient education. The APP plays a critical role in delivering high-quality healthcare services and ensuring excellent patient outcomes. Job Requirements, Prerequisites and Essential Functions

Pay Range: $87.58 - $102.18 - $119.77 Essential Functions and Responsibilities

Assess patient status by obtaining health history, evaluating presenting illness, risk factors, family history, psychosocial factors, and cultural factors; perform physical examinations. Order/perform appropriate laboratory and diagnostic tests; consult with physicians and other clinicians as needed; integrate data to determine diagnosis and therapeutic plan; identify health risks. Develop and implement treatment plans by prescribing/dispensing medications and/or injections in compliance with guidelines and state laws; educate patients and families; coordinate case management information; document in EMR. Participate in peer reviews, chart reviews, staff education, guideline development, continuing education, and quality assurance activities; act as consultant to clinical team; ensure patient confidentiality; advocate for patients. Collaborate with physicians to manage acute and long-term medical needs; provide continuity of care and triage patient calls/e-mails. Perform other duties as required or assigned. Assessment

Elicit and document complete health history; perform and document comprehensive physical examination; order appropriate diagnostic tests. Assess patient perception of health status, personal strengths, support systems, and psychosocial, physical, environmental, economic, and cultural factors. Identify patients and families at risk for specific healthcare problems. Diagnosis and Planning

Formulate diagnoses based on history, physical, and diagnostic tests. Develop health care plans with patients and families, including treatment, health maintenance education, and counseling. Establish and document short- and long-term goals in collaboration with each patient. Plan comprehensive patient care with other health care providers. Implementation

Provide appropriate care based on patient age and needs; implement treatment plans based on assessments and test results. Counsel patients on motivation and adherence; provide telephone advice and triage as needed. Document all pertinent data and interventions in patient records; provide medication refills as appropriate; review abnormal lab data and tests and take action; consult with other providers as indicated. Evaluation

Evaluate patient understanding and adherence; re-evaluate goals with patients and health professionals. Education

Patient and Family: identify learning needs, assess readiness, provide individualized health education, and develop teaching goals. Staff: function as a clinical resource person. Continuity/Coordination

Serve as the health care provider for a specified patient group; coordinate care among providers, patients, and families; participate in discharge planning when appropriate; maintain ongoing communication with community agencies to facilitate continuity of care. Advocacy

Promote patient and family participation in planning; provide information to make informed decisions; inform patients of their rights; provide referrals to community resources as appropriate. Education and Qualifications – Physician Assistant

Graduation from an accredited Physician Assistant program Certification by the National Commission on Certification and licensure in California License/Certifications

Active and unrestricted Physician Assistant license in California Current national board certification and active, transferable DEA license as required Eligible for payer enrollment with CMS/Medicare and Medicaid BLS and ACLS (American Heart Association) required Experience
