We are seeking a patient-focused
Clinical Pharmacy Specialist I
to join our
Palliative Care team
in DC. In this role, you’ll provide advanced, direct patient care pharmacy services to a specialized patient population, collaborating closely with providers, nurses, patients, and their families. You’ll serve as a subject matter expert, mentor clinical pharmacists, and help shape clinical practice standards within Palliative Care. Key Responsibilities
Evaluate therapeutic options and provide evidence-based clinical guidance to pharmacists, residents, and staff. Review and verify medication orders for accuracy, resolving discrepancies with physicians, pharmacists, and nurses. Develop individualized patient care plans, counsel patients on medication use, and participate in interdisciplinary patient care rounds. Lead efforts in continuous quality improvement, ensuring compliance with clinical guidelines and minimizing non-formulary drug use. Author and present evidence-based documents such as newsletters, drug monographs, therapeutic interchanges, and shortage mitigation strategies. Serve as a
preceptor and mentor
to pharmacy residents and students on clinical rotations. Provide leadership and functional supervision for support personnel, managing workflow, resources, and productivity standards. Qualifications
Must Have: Doctor of Pharmacy (PharmD) or equivalent from an accredited college of pharmacy. Active pharmacist license in Washington, DC. Minimum 3 years of hospital experience with at least 4 years in a specialized area
OR
completion of PGY-1 and PGY-2 residency
OR
completion of PGY-1 plus 2 years of direct patient care experience in a specialized area. Nice to Have: Palliative Care experience strongly preferred. Bilingual communication skills (not required).
