What You'll Do:

• Deliver comprehensive primary care to a diverse group of patients
• Collaborate with a multidisciplinary team that includes care coordinators, behavioral health specialists, and pharmacists

Required Qualifications:

• Unrestricted medical licensure in Indiana or eligibility to obtain prior to employment
• Board certification or eligibility in Family Medicine or Internal Medicine or Med-Peds
• Active and unrestricted DEA License or ability to obtain before starting
