The agency, through its programs and services, encourages the inclusion of people with disabilities into the mainstream of society, including social, recreational, family and work activities.A community based Enrollment Specialist for our Pennsylvania Self Directed Program will educate participants and their direct care workers on how to successfully enroll and utilize services under the Community HealthChoices Participant-Directed Services Program. Essential Job FunctionsAccept and outreach to participant within 3 days of referral to determine how the packet will be delivered to the participant: mail, email or in personProvide in-person participant orientation within 15 days of referralTrain participants and direct care workers the skills needed to enroll and manage this self-directed programMandated ReportingWorker Safety TrainingIndependent Living PhilosophyAssist participant and direct care worker with enrollmentCommon Law Employer (CLE) enrollment paperwork including CLE Roles and ResponsibilitiesAssist with DCW enrollment paperworkDCW Pre-Service OrientationUnique IDBackground check clearancesProvide training on EVV and timesheet submission/approval processAttend in-service, supervision and staff development meetings when requested by managerDemonstrate a working knowledge of program policies and procedures and self-directed regulationsDemonstrate a commitment to the Independent Living philosophy of consumer controlMusty report all suspected incidents of fraud, waste, and abuseTrack for receipt of required documentation within required timelines:Enrollment documentationReturn all phone calls from participants and DCW within 24 hoursWork within policies to complete and submit internal requirements: Contact NotesDayforce, Outlook Schedule Mileage and Expense FormsFollow up on requests for additional skills training by the Fiscal Management Services (FMS):Non-billing, overbillingMajor problems requiring skills trainingParticipant status updatesAll other duties as assigned by the Skills Trainer ManagerQualificationsOne year experience providing services for persons with disabilitiesExperience or education in teaching is helpfulKnowledge of community resource that participants are involved withExcellent communication, organizational, and writing skillsBasic computer skills in Microsoft Office/Outlook and the ability to navigate databasesRequired EducationBachelor’s Degree preferred; GED or High School Diploma required Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.While performing the duties of this job, the employee is regularly required to talk and/or hear.
