Duties & Responsibilities

Purpose of Position :

Under the direction of supervision of Physical Therapy, provides home visits for diagnostic and therapeutic physical therapy services to home health patients. Examines and evaluates the patient's condition and plans and administers treatments to promote optimal health. Seeks to relieve pain, improve the body's movement and function, maintain cardiopulmonary function, restore, maintain and promote optimal physical function and limit disabilities resulting from injury or disease. Acts in compliance with applicable local, state, and federal rules and regulations and physical therapy standards of care.

Essential Duties and Responsibilities : 1. Provides physical therapy services, in accordance with the physician's plan of care. 2. Assists the physician in assessing the patient's functional level by applying appropriate tests and measurements. 3. Evaluates the patient and establishes/upgrades the plan of treatment by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests. 4. Treats the patient through the use of therapeutic activities to relieve pain, develop and restore function, attain and maintain maximum performance and utilize adaptive equipment within the functional limitations of the patient and in accordance with the plan of care. 5. Regularly observes signs and symptoms of the patient's response to treatment and rehabilitation needs. Documents and reports findings to the physician and/or other appropriate health professional. 6. Notifies the coordinator of care and/or the physician of the patient's development and the need for revisions to the patient's plan of care. Ensures that the plan of care addresses all problems identified during the evaluation or demonstrates rationale for not doing so. 7. Develops, prepares and maintains individualized patient care documentation with accuracy, timeliness and in accordance with the System's policies. 8. Assists with the coordination of the plan of care and maintains continuity of patient care through interdisciplinary coordination with other health professionals by attending patient care conferences. Makes referrals to other disciplines, as indicated by the needs of the patient or document rationale for not doing so. 9. Evaluates the home environment and makes appropriate recommendations with regard to safety issues. 10. Orders and/or fabricates adaptive/assistive devices and equipment and trains the patient in the safe and effective use of the equipment. 11. Instructs, trains and advises the patient, family members and other home health personnel in the phases of physical therapy in which they may assist the patient, and in the safe and effective use of equipment, assistive devices, and the completion of the home exercise program. 12. Demonstrates and teaches alternate techniques to perform activities of daily living, transfers, and mobility and positioning. 13. Teaches, supervises and counsels the patient and designated caregiver regarding home care procedures, as appropriate to the patient's condition.

Minimum Qualifications

New York State license and current registration to practice as a Physical Therapist; and

One year of post license experience as an Physical Therapist.

Holds, or obtains, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
