Job Duties:
Conduct visits shortly at beginning of a patient's home health episode
- Perform intake assessments and support appropriate documentation for transitions
- Assess for clinical stability and determine if urgent or early primary care follow-up is needed
- Summarize recent care history and current clinical status
- Document clinical findings in the EMR to update care records and flag any clinical concerns
- Provide education and emotional support to patients and their families
Qualifications
- High emotional IQ
- Strong clinical assessment skills and comfort practicing independently
- Minimum of 2-5+ years clinical experience, include at least 1 year practicing as an APP
- Experience providing care in the home preferred
- Exemplary communication and teamwork skills
Compensation
Base: $115+
