Description Conduct thorough evaluations for patients in either inpatient or outpatient settings and tailor personalized treatment plans Implement physical therapy interventions focused on enhancing mobility, boosting strength, improving balance, and maximizing functional outcomes Collaborate seamlessly with nursing staff, physicians, case managers, and fellow rehabilitation professionals to drive optimal patient success Guide patients and their families through educational resources on therapeutic objectives, discharge planning, and at-home exercise routines Document care evaluations, treatments, and patient progress meticulously, aligning with facility and regulatory standards Engage actively in care team discussions and interdisciplinary conferences to maintain cohesive patient care Education

Doctorate in Physical Therapy (DPT) from an accredited institution or equivalent degree Certifications

Current state licensure as a

Physical Therapist CPR certification is preferred Additional specialized certifications (e.g., geriatric, neurological) appreciated but not mandatory Skills

Prior experience is strongly desired Ability to address complex medical conditions and mobility challenges Excellent communication and collaboration skills are essential Proficient in utilizing digital charting and clinical documentation tools Benefits
