In-jail services include medical, nursing, mental health, substance use treatment, social work, dental and vision care, discharge planning, and reentry support. In addition to providing direct patient care in the jails, CHS leverages the resources of the nation's largest municipal health care system to help discharged patients successfully return to their communities. CHS is also a pivotal partner in New York City's criminal justice reform efforts.
Work Shifts
Time: 4:00 PM - 12:00 AM Days: Sun, Mon, Tue, Wed, Thu Facility: EMTC at Rikers Island
Duties & Responsibilities
This full-time position is a part of the Community Connections Service (CCS) team, within the Reentry and Transition Services Department. CCS consists of a team of jail-based staff who assist patients recently admitted to jail, with benefits applications such as Medicaid and obtain consents for attorney and community connections. This position is based on Rikers Island and is a direct service role.
This position requires excellent communication skills, basic computer skills, resourcefulness, organizational skills, comfort in navigating jail environments, and adaptability. Ideal candidates will be friendly and have experience working in a correctional setting.
Responsibilities
Greet patients newly admitted to NYC jail system Obtain voluntary patient consents (e.g., HIPAA) Scan and upload documentation to patient's chart in the electronic health record Maneuver within the jail-based clinics and/or housing areas to complete services Additional tasks and projects as requested by CHS/RATS leadership Minimum Qualifications 1. A four-year high school diploma or its educational equivalent and four (4) years of satisfactory full-time experience providing direct counseling, guidance, crisis-intervention or information and referral services for inmates of a correctional facility, substance abusers or a similar client population; or 2. Education and experience equivalent to "1" above. Thirty (30) semester credits from an accredited college will be considered equivalent to one (1) year of the required experience. However, all candidates must have a four-year high school diploma or its educational equivalent and at least one (1) year of experience as described in "1" above.
