Position Highlights: Position: Patient Navigator Location: 1200 S York Rd. Elmhurst, IL Full Time 36 hours Hours: Monday-Friday 7:30am-4pm Required Travel: no What you will do:
Greet and welcome patients and their families, offering assistance and guidance. Provide a warm and compassionate presence, addressing patient questions and concerns with empathy. Act as a point of contact for patients and their families, ensuring they feel comfortable and supported throughout their healthcare journey. Assist patients in scheduling appointments with healthcare providers, specialists, and other necessary services. Coordinate appointment reminders and follow-up as needed. Communicate appointment details and instructions clearly to patients. Provide information on available hospital services, community resources, and support programs. Assist patients in accessing financial assistance programs and insurance information. Collaborate with social workers and other healthcare professionals to connect patients with appropriate resources. Maintain accurate and up-to-date records of patient interactions, services provided, and referrals made. Input data into electronic health records and hospital databases as required. Advocate for patients' rights and preferences within the healthcare system. Assist in resolving patient concerns and issues in a timely and effective manner. Collaborate with the Patient Navigation Coordinator to address complex patient needs. Work closely with the healthcare team, including nurses, social workers, and case managers, to ensure seamless patient care. Attend team meetings and contribute to patient care planning discussions. High School Diploma or GED - required Strong problem-solving skills Ability to function in an organized and time conscious manner Minimum one year previous business office experience/clinic experience Current CPR Certification issued by American Heart Association or American Red Cross - Required Current certification or registration through American Association of Medical Assistants (AAMA), American Registry of Medical Assistants (ARMA), or any other certifying agency - Strongly Prefered Previous insurance and managed care experience - Preferred Previous clinical experience - Strongly Preferred Prior authorization or procedure scheduling - Preferred Benefits (For full time or part time positions):
