The Adult outpatient substance use treatment program (ASATS) specializes in providing culturally responsive services in several threshold languages like Vietnamese, Spanish, Ilocano, and Mandarin. ASATS provides individual counseling and an array of counseling groups that focuses on specific challenges like co-occurring disorders, criminal-justice impact, and many more.
Hours of operation are Mondays and Fridays from 9:00AM to 6:00pm; Tuesdays, Wednesdays, and Thursdays from 9:00AM to 8:00PM.
Responsibilities
Individual Treatment Responsibilities:
- Provide pre-admission and intake services to clients, as well as individual peer support to engage clients throughout treatment, while providing support in preparation for discharge.
- Facilitate and support clients’ movement throughout the program with particular attention to ancillary resources and progress toward meeting goals outlined in care plans.
- Facilitate orientations, coordinate care with client and key stakeholders.
- May be required to assist in providing client education.
- Proactively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status.
- May provide support for clients at external appointments. Track and implement incentives/sanctions matrix.
- Consult with clinical supervisor and participate in both group and individual clinical supervision.
- Participate in development of discharge plans and levels of care including extension requests based on need.
- Regularly assist Care Coordinators to follow up on urinalysis tests results, if applicable.
Treatment Setting Responsibilities:
- Coordinates Peer Support/Continuing Care/Peer Mentor led groups.
- Conducts community outreach and education workshops.
- Attends required trainings and meetings.
- Plans and facilitates client celebrations and special events as assigned (i.e. Completion Ceremonies, Pro-Social activities and Family Nights).
- May be asked to work on a weekend or during after-hours for crisis stabilization, community education, resource fairs, and special events.
- In addition to providing services at the agency’s main site, services may be provided off site in order to assist in engaging clients to treatment.
- Possibility of home visits and/or in-custody peer support services and case management.
- Driving clients to support with clinical needs/or participate in rehabilitative activities (i.e. driving client to social services department or driving client to a social recovery event while provide peer support).
Documentation Responsibilities:
- Collaborate with Case Manager, Care Coordinator, client and other available internal and external resources to assist in developing/maintaining care plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client as appropriate. Properly document all peer support services sessions in a timely manner.
- Maintain accurate records by entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
- May participate in Peer Chart/ Utilization Reviews for preparation of program audits as needed and/or appropriate.
- Be responsive and proactive in all correspondence with referring parties, judges, probation and parole officers, social workers, and the Quality Improvement Dept. via treatment status reports, phone calls, faxes, etc. as appropriate.
- Will work with Case Manager or Clinician to provide informed, written status reports to the above entities as appropriate.
- Will assist in providing written progress reports/narratives to Clinical Program Manager or Directors as requested.
Outreach Responsibilities:
- Refer individuals to HealthRIGHT 360 for substance use and mental health services.
- Provide direct community outreach and health educational workshops, groups or programs to targeted populations.
- Participate in health fairs, neighborhood community activities, screenings, and special events.
- Assist in the planning and coordination of community health fairs and events such as Sister 2 Sister.
- Assist in the development and implementation of a marketing campaign to the community served by HealthRIGHT 360.
- Maintain comprehensive documentation for data collection; and provide statistical data and/or reports per grant requirements.
- Meet or exceed outreach performance measures expectations.
Productivity And Coverage Responsibilities:
- Achieve service delivery goals as set forth by the program requirements.
- May be required to meet service delivery goals at other Santa Clara County Behavioral Health programs.
- Ensure coverage during absences or unplanned events by coordinating with team members and management.
- Adjusts workload as necessary to ensure uninterrupted service delivery, including stepping in to provide support during peak demand times or staff shortages.
- Adheres to the program’s scheduling protocols to maintain appropriate client-to-provider ratios.
Qualifications
Education, Credentials, and Experience
- High School Diploma, GED, or equivalent accreditation required. Bachelor’s degree in counseling, Social Work, or related field preferred.
- Valid California driver’s license.
- Lived experience like that of targeted population preferred.
- Peer Certification required, pending Peer Certification will be considered
- Current registration or certification with a Drug and Alcohol Certification program recognized by DHCS preferred.
- Experience working within the Santa Clara County –Department of Behavioral Health Services system preferred.
Background Clearance
- Must not be on active parole or probation, clear with OIG database.
- Must complete a background check and live scan.
