Licensed as a Social Worker in state of practice is preferred and required within 6 months of hire.
Responsibilities
- Provides individual and family psychotherapy
- Group facilitation; participates in family and treatment team meetings
- Discharge planning
- Community resource knowledge and collaboration
- Psychosocial assessments
- Knowledge of involuntary processes; Knowledge of guardianships, advanced directives, and POAs
- SUD screenings and SUD discharge planning
- Preadmission Screening and Resident Review (PASRR) completion
- Post-discharge calls
- Administer and document patient health questionnaires and screenings including PHQ-9, C-SSRS, and SDOH
- Crisis assessment and referral services
- Participation in required training for Bon Secours Mercy Health and departmental programs
Qualifications
- Education: Bachelor’s in Social Work (required); Master’s in Social Work or Social Administration (preferred)
- Licensing/Certification: Licensed as a Social Worker in state of practice (preferred; required within 6 months of hire)
- Work Experience: 1 year in a social or human services setting (required)
