Licensed as a Social Worker in state of practice is preferred and required within 6 months of hire.

Responsibilities

• Provides individual and family psychotherapy
• Group facilitation; participates in family and treatment team meetings
• Discharge planning
• Community resource knowledge and collaboration
• Psychosocial assessments
• Knowledge of involuntary processes; Knowledge of guardianships, advanced directives, and POAs
• SUD screenings and SUD discharge planning
• Preadmission Screening and Resident Review (PASRR) completion
• Post-discharge calls
• Administer and document patient health questionnaires and screenings including PHQ-9, C-SSRS, and SDOH
• Crisis assessment and referral services
• Participation in required training for Bon Secours Mercy Health and departmental programs

Qualifications

• Education: Bachelor’s in Social Work (required); Master’s in Social Work or Social Administration (preferred)
• Licensing/Certification: Licensed as a Social Worker in state of practice (preferred; required within 6 months of hire)
• Work Experience: 1 year in a social or human services setting (required)
