Overview

Permanent, full-time Social Worker to join the West Bedford Transitional Health team based in Bedford, Nova Scotia. The Social Worker supports the goal of returning to home/community through interprofessional collaboration, providing evidence-based clinical expertise to inform care plans aligned with patient and family goals, effective communication, shared decision-making, and mutual support.

The Social Worker focuses on relationships between people and their environments, supporting psychosocial, emotional, and spiritual wellbeing. Build on patients’ and families’ existing coping strategies and problem-solving skills, past successes, and personal resources. Contributes to ongoing quality improvement, learning, mentoring, and the exploration of new approaches.

Responsibilities

  • Complete initial and ongoing bio-psychosocial assessments to identify strengths, challenges, and psychosocial risks influencing health outcomes.
  • Develop restorative action plans with individualized interventions addressing funding needs, housing, resources, trauma, crisis, psycho-educational needs, addictions, grief and loss, stress management, life transitions, and mental health issues.
  • Facilitate psycho-educational activities/groups for patients and families to increase self-care and self-management.
  • Provide orientation to students, develop and deliver presentations on pertinent topics, and offer formal and informal peer mentorship.
  • Develop collaborative partnerships with community and government partners to address systemic barriers and enhance health and wellness for patients, families, and stakeholders.

Qualifications

  • Bachelor of Social Work
  • Current registration with the Provincial Association of Social Workers
  • Minimum of two years of clinical experience
  • Strong communication to motivate, persuade, and negotiate, including handling sensitive or controversial issues with probing, investigation, and explanation
  • Asset: Previous experience supporting admission and discharge of vulnerable populations and advising on care plans; Masters in Social Work
