Enrollment/UAS Assessment Nurse

Responsibilities
Conducting pre-enrollment assessments for prospective enrollees utilizing clinical nursing expertise and critical thinking skills including obtaining preventive health service data.
Completes UAS - New York Assessment instrument to determine continuing eligibility and / or identify any needed changes to Care Plan.
Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and will need to develop a prospective plan of care.
Communicates with the Care Team and participates in interdisciplinary team meetings.
Assessments will be conducted in The Bronx. Reliable transportation required.
Responsible for environmental assessments.
Completes semi-annual assessments as requested
Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
Conducts chart audits as requested if time permitting
Qualifications
Current NY State Registered Nurse License
Education:
Bachelor’s Degree in Nursing
