Travel Job Description American Traveler is seeking a Physical Therapist with active CA licensure and home health experience for a day shift assignment within multiple counties. Responsibilities
Home health setting serving patients in Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville Day shift schedule Assignment length is 13 weeks Productivity measured in points: most visits equal 1 point (1.33 hours), OASIS SoC equals 1.5 points (2 hours) Expected to travel within and occasionally beyond the assigned counties to meet caseload and visit requirements Weekend coverage and additional travel may be required to maintain visit expectations Primary duties include conducting evaluations, discharges, routine visits, re-evaluations, OASIS SoC, RoC, and ReCert visits Documentation, drive/travel time, and phone calls to patients and doctors are included in the work expectations Travelers must complete weekly visit point goals to maintain productivity and guarantee eligibility Travelers must be willing to see COVID+ and COVID PUI patients Smartphone and standard home health medical devices are provided for clinical use Former employees are not eligible for travel assignments All requested time off (RTO) must be disclosed upon application; additional time off after acceptance will not be approved Professional attire and use of required PPE as needed Requirements
Current, active CA Physical Therapist license (license required if applicable) Minimum 1 year of recent physical therapy experience required Current BLS certification required Valid driver's license, auto insurance, and auto registration required Annual TB test required Vaccinations required: MMR, Varicella, HepB, TDap (within the last 10 years), annual Flu vaccine Pre-employment drug screen and physical required Benefits
