Salary: Base $280,000-$300,000 Hours: Monday – Friday; Clinic Hours 7:30a–5:00p (last patient scheduled at 4:15p) Work / Life Balance Small Patient Panel: 60-90 patients max Manage PCPs and NPs Patient Population: Seniors with 3-5 chronic diseases Value Based Care Center Medical Director Training

Inhouse Consulting Specialist: Cardiology, Podiatry and more Holistic Health Services: Acupuncture, Tai Chi, Yoga and Nutrition Door to Door Transportation for our patients (Uber / Lyft) Requirements
Board Certified / Board Eligible in FM, IM or Geriatric Medicine Education: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty. Experience: At least 1 year of clinical experience in a geriatric or family practice setting. Leadership experience preferred Skills: Strong leadership, communication, and analytical skills.
