The Division of Endocrinology, Diabetes, and Nutrition is currently recruiting a Physician Assistant to be a part of the Ambulatory Diabetes Clinic as part of the Glucose Management Team. The Glucose Management Team is comprised of a Medical Director, Physician Assistants, Certified Diabetes Nurse Educator, Pharmacist, and Social Worker. The team will manage inpatient glucose levels, provide education, assist in social needs and transitioning patients to the ambulatory care setting at discharge. As many as 1/3 hospitalized patients have hyperglycemia or underlying diabetes. This program aims to provide value-based care and improve the long-term health outcomes of these patients. We are looking for energetic, team-oriented individuals interested in improving health outcomes, health literacy, and social barriers in an inner-city population.

Under physician supervision, the position for the Physician Assistant (PA) in the Glucose Management Team will provide care for discharged patients from inpatient to ambulatory care settings. By working directly in the ambulatory setting, the PA streamlines the discharge process, facilitates timely follow-ups, and supports the delivery of value-based care aimed at improving long-term health outcomes.

Department Essential functions:

Perform initial evaluation and physical exam for patients with new onset or existing diagnosis of diabetes

Provide consultation services to inpatient primary teams regarding inpatient glucose management plans during the hospital stay

Provide appropriate outpatient treatment recommendations to ensure optimal glycemic control

Participate in daily multidisciplinary rounds with endocrinology attendings and discuss patient care

Provide education to patients and their families about their diagnosis

Collaborate with a multidisciplinary team to ensure a smooth transition to outpatient care

Attend weekly endocrinology grand rounds and core conferences

Education and/or Experience

Graduation from an accredited Physician Assistant Training Program approved by the Board of Physician Quality Assurance is required.

National Commission on Certification of Physician Assistants (NCCPA) required by the time of hire.

Licensure by Maryland State Board of Physicians as a Physician's Assistant is required at the time of hire.

Current Basic Cardiac Life Support Certification is required.

Ability to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit/area/department policies and procedures.

Highly effective verbal and written communication skills are required to interact with patient families, departmental units, medical and nursing staff on all essential matters. Demonstrated/documented effective interpersonal skills.

Total Rewards

The referenced base salary range represents the low and high end of University of Maryland’s Faculty Physician’s Inc. salary range for this position. Some candidates will not be eligible for the upper end of the salary range. Exact salary will ultimately depend on multiple factors, which may include the candidate's geographic location, skills, work experience, market conditions, internal equity, responsibility factor and span of control, education/training and other qualifications. Qualifications

Education: See above under Education and/or Experience.

Experience: Prior PA clinical experience in endocrinology, diabetes, or ambulatory care is preferred.

Licenses & Certifications: Maryland Physician Assistant license; NCCPA certification; BCLS certification.
