Primary Care Plus (PCPlus), a value-based care delivery organization, is seeking Board-Certified/Board-Eligible Internal Medicine or Family Medicine Physicians to join our growing team across Louisiana. Our mission is to provide a path of well-being for patients through high-quality, cost-effective, and coordinated care that delivers superior outcomes and satisfaction. Responsibilities
Provide primary care services in a value-based care setting for adults in a community clinic Deliver comprehensive preventive, acute and chronic disease management with 30-60 minute appointments; see 10-16 patients/day Collaborate with APRNs, RNs and a dedicated Quality Improvement team to ensure high-quality care Engage in care coordination and outreach programs to support patient health outcomes Operate without overnight on-call responsibilities and support a 4.5-day work week with early Friday departures Compensation & Benefits

M.D. or D.O. Board Certified/Eligible in Family or Internal Medicine Active, unrestricted Louisiana medical license Valid DEA and CDS licenses (or ability to obtain) Preferred Qualifications
