The RN Field Clinical Care Coordinator will be an essential element of an Integrated Care Model by relaying pertinent information about members’ needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. This is a Field-based role. If you reside within a commutable distance to the Boston market in the listed areas, you will have the flexibility to work telecommute as you take on some tough challenges. This is a Field-based role. Expect to spend about 50% of your time in the field visiting our members in their homes or in long-term care facilities. Primary Responsibilities Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment, including medical, behavioral, functional, cultural, and socioeconomic needs Assess, plan and implement care strategies that are individualized by patients and directed toward the most appropriate, least restrictive level of care Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services Manage the care plan throughout the continuum of care as a single point of contact Partner and collaborate with internal care team, providers, and community resources/partners to implement care plan Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members Reasons to consider working for UnitedHealth Group Competitive base pay and comprehensive benefits Paid time off and holidays Medical, dental, vision, disability and life insurance options 401(k) and Employee Stock Purchase Plan Education reimbursement and employee discounts Employee assistance and referral programs Voluntary benefits (pet, legal, LTC insurance, etc.) Required Qualifications Current and unrestricted independent licensure as a Registered Nurse in the state of MA 1+ years of clinical experience Intermediate proficiency with MS Office (Word, Excel, Outlook) Access to a designated quiet workspace at home with ability to secure PHI Reside in an area with UnitedHealthcare approved high-speed internet or have existing service Ability to travel to visit members in homes or other settings (community centers, hospitals, providers’ offices) Access to reliable transportation and valid US driver\'s license Preferred Qualifications Bachelor’s or Master’s Degree in Nursing Certified Care Manager (CCM) 1+ years of community case management experience for individuals with complex needs Experience in team-based care Background in Managed Care Experience with Electronic Medical Records or similar platforms Bi-lingual in Spanish, Cantonese, or Mandarin Physical Requirements Ability to transition between office and field locations Ability to visit members and providers across multiple locations Ability to transport equipment (e.g., laptop) to field locations Ability to remain stationary for computer or tablet work Telecommuting eligibility per UnitedHealth Group policy Pay and benefits Pay is based on factors including location, education, experience, and certifications. The hourly pay range for full-time employment is $28.27 to $50.48. We offer a comprehensive benefits package and other incentives, subject to eligibility requirements.
