Doctorate in dentistry from an accredited program approved by the Council on Dental Education of the ADA Experience as a General Dentist of at least 12 months within the preceding 36 months; newly graduated dentists also considered Successful completion of at least 12 classroom hours of continuing dental education Possess a current, unrestricted license to practice dentistry in any one of the fifty States, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands For More Information Contact
