Geriatrics Nurse Practitioner

Overview
Seeking two full-time Board-C Certified Nurse Practitioners who are passionate about providing quality care to the geriatric population in northeastern Massachusetts. The role involves participation as a member of the PACE interdisciplinary team in planning, implementing and evaluating care plans that meet objective standards and policies of the PACE model of care, and collaboration with site Medical Doctors to provide comprehensive primary care to participants. Schedule: Monday through Friday, 8:00 am – 4:00 pm. No nights, weekends, or holidays. Fluency in Mandarin and/or Khmer is required for one designated role. Responsibilities
Conduct initial examinations of all new participants and quarterly exams during the first year of enrollment. Evaluate participants for episodic visits, acute illnesses, and follow-ups of hospitalizations. Perform nursing home visits in accordance with state nursing home requirements and as needed. Provide home visits as needed for acutely ill or chronically ill participants who are homebound. Participate in After Hours On-Call Coverage. Provide health care teaching to patients, families, caregivers, and other members of the Interdisciplinary Team. Adhere to the standards in the Collaborative Practice Guide and complete appropriate, accurate, timely documentation onsite and offsite. Ensure duties are accomplished within budgetary constraints and interactions with clients uphold high standards of care. Must be able to pass a fit test and wear a respirator mask when required (facial hair must not interfere with a tight seal). Frequent local travel may be required. Performs other duties as assigned. Qualifications
Master of Science Degree in Nursing (MSN). Current Board Certification as a Nurse Practitioner in the Commonwealth of Massachusetts. Massachusetts and DEA Prescription Privileges. Credentialed for the affiliate Long Term Care Facilities. Current CPR Certification. Minimum of one (1) year working with a geriatric population. Ability to convey information clearly and succinctly to all employees, and to work effectively in a team. Ability to lead independently and collaboratively with others; attention to detail and timely task completion. Ability to recognize situations early and implement effective solutions.
