Overview
Position: Temporary Discharge Manager. At Stony Brook Medicine, a Temporary Discharge Manager is a valuable member of the team who assists with transitional office operations. The individual works closely with Nurse Case Managers and Social Workers regarding payer delays, authorization and denial notification, and serves as a liaison between hospital departments to improve department function and initiatives. The role supports discharge planning efforts, coordinates trainings and meetings for the care management staff, and maintains departmental resources for staff. Responsibilities
Partner with Senior Management to define project scope, establish goals, objectives, and timelines. Work closely with nurse case manager/social worker teams regarding payer delays, authorization and denial notification. Serve as a liaison between hospital departments to support department function and initiatives. Assist with transitional office operations including post-acute authorization and transportation coordination. Prioritize PT/OT consults daily based on discharge readiness and authorization requirements. Maintain timely and up-to-date project tracking. Manage complex projects with multiple stakeholders. Maintain a high level of customer service. Maintain and coordinate departmental resources. Develop competency and maintain up-to-date knowledge of hospital software (e.g., PowerChart, Siemens, Eclipsys, and other programs as acquired). Coordinate trainings and meetings; maintain understanding of referral resources. Support discharge planning efforts and facilitate staff education as needed. Qualifications
Required Qualifications: At least 3 years of prior experience in an academic medical center/clinical setting in an administrative and/or supervisory capacity. Demonstrate superior written and oral communication skills. Computer proficiency in Electronic Medical Record applications. Knowledge of discharge planning and length of stay. Leadership skills, ability to run meetings, rounds, and educate staff. Strong customer service skills. Patient-first mindset, patient advocacy, and service excellence. Ability to project manage, plan actions, and work as a team leader. Ability to cover multiple patient populations. Weekday and weekend flexibility. Preferred Qualifications: NYS LPN License or other advanced clinical education. Bachelor’s degree in health science or related field. Advanced Degree/Certificate. Hospital operation and throughput experience. Leadership experience working with C-Suite executives. Experience in Care Management and in Managed Care/Insurance industry/MD office. Working knowledge of medical terminology. Knowledge of regulatory requirements (CMS, EMTALA, Managed Care, etc.). Experience with post-acute care coordination including acute rehab/skilled nursing and transportation authorizations. Special Notes
