Required Qualifications : Bachelor's Degree in PHARMACY from an accredited School of Pharmacy. Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience or equivalent education/experience. Oregon Pharmacist License upon hire. Within 2 years of hire: Board Certified by the Board of Pharmacy Specialties specific to area of practice. Pharmacy preceptor license in applicable state as applicable upon hire. Two years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting OR five years of pharmacy practice experience, with at least two years in a relevant setting in lieu of PGY-1 and PGY-2 residency requirements.
