A physician-owned multispecialty group in Fall River, Massachusetts, is seeking a dedicated Nurse Practitioner to join its team providing care to homebound patients and residents in local skilled nursing facilities (SNFs). Position Overview:
As part of the Home Care program, the Nurse Practitioner will conduct house calls, reconciling medications, educating patients and caregivers, diagnosing conditions, ordering appropriate testing, and coordinating care with visiting nurse agencies, primary care providers, and specialists. Patients are referred by PCPs, hospitals, and case managers, with visiting nurses providing daily visits and APPs serving as backup resources.
In the SNF setting, the Nurse Practitioner will round in local facilities alongside other NPs and facility medical directors, directing patient care plans, managing transitions, and supporting efforts to reduce hospital readmissions and shorten facility length-of-stay.
Qualifications:
- Current Nurse Practitioner license in Massachusetts (Rhode Island licensure a plus)
- Massachusetts controlled substances registration
- 2+ years of clinical experience preferred (geriatrics, home care, or SNF settings a plus)
- Strong organizational skills, independence, and clinical judgment
- Ability to collaborate effectively with nurses, therapists, case managers, and physician colleagues
- Fluency in Portuguese is a plus but not required
