Job Summary:  

MedStar Health is looking for  a   Behavioral Health Therapist   t o  join  our   HEART Team !   We are looking for   a licensed social worker with e xcellent oral communication, writing, organizational  skills  and abilities , and  utilization  of  approaches and strategies that support an d  optimize  patient care outcomes that are based on the interdisciplinary plan of care.    

The primary responsibility of the Behavioral Health Therapist- HEART Team is to deliver behavioral health counseling to beneficiaries served by the HEART Team. The HEART Team provides holistic, interdisciplinary, community based medical and behavioral health services designed to help beneficiaries manage complex medical and behavioral health illnesses. It aims to restore health, improve community, daily living, personal, and social skills while reducing incidence of medical or behavioral health crises that lead to unnecessary use of the hospital system. The Behavioral Health Therapist-HEART Team is responsible for providing ongoing psychological assessment and supportive counseling to minimize the negative effects of psychiatric symptoms and/or substance dependence that interfere with the beneficiary's ability to manage their health, daily living activities, and personal development, The Behavioral Health Therapist-HEART Team is also responsible for providing clinical supervision to assigned Peer Recovery Specialists.

Primary Duties:  

  • Provides  individual, family and group psychotherapy services based on an understanding and integration of various theoretical approaches to human behavior .   Collaborates with HEART Team members and beneficiaries to develop therapeutic goals and interventions.  Develops wellness plans, crisis plans, safety plans, relapse prevention plans as needed to support the maintenance of stable functioning.   

  • Participates in regularly scheduled caseload consultation with the supervisor and/or HEART Team and communicates behavioral health recommendations to the beneficiary and the beneficiary's care  providers. Communicates  with HEART Team primary care providers, psychiatrists, nutritionists, vocational specialists, care  managers , community health  workers  and other care providers including external agencies.  Documents work according to standards in diagnostic evaluations, clinical assessment, progress notes, discharge procedures, etc.   In consultation with HEART Team, recommends plan changes for beneficiaries who are not improving as expected   

  • Determines  use of Peer Recovery Specialists and  provides  clinical guidance on their services.   Works closely with other clinical/professional staff to  maintain  communication and  provide  feedback   

  • Participates in education and other activities necessary to  maintain  competency in clinical work.   Participates in 24/7/365 on-call rotation and coordinates after-hours resources .  

  • Provides  services remotely, in-office, telephonically, via telehealth, in the community, or beneficiary's home as beneficiary needs and preferences dictate.  Remains solution focused .  

Qualifications:  

  • Master’s degree Psychology, Counseling, Social Work  

  • LCSW-C - Licensed Certified Social Worker - Clinical Any of the identified licenses meet the requirements for this position. or LPC-Licensed Professional Counselor or Licensed Psychologist or LCPC-Licensed Clinical Professional Counselor or  L icensed  Grad Prof Counselor  –  LGPC .

This position has a hiring range of $60,632 - $107,494
