The Integrated Care Center Clinic is a Federally Qualified Health Center that provides primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need, including those who are suffering from homelessness, mental illness, substance use disorders, and chronic medical conditions. We provide team-based care in an integrated care setting. About The Behavioral Health Department The Behavioral Health Team offers an opportunity to grow practice experience in individual and group settings. Our patients span the full range of mental health disorders, from life stressors and trauma to schizophrenia, personality disorders, addiction, and complex PTSD. We offer supportive clinical supervision, group consultation, peer support, interdisciplinary training, and a generous training budget to advance your skills. The Integrated Care Center fosters collaboration between Primary Medical Care, Mental Health Services, Substance Use Disorder Counseling, Residential Treatment and Addictions Medicine, Dental Care, and Case Management departments for integrated care and better client outcomes. Job Overview: Behavioral Health Clinician Key Responsibilities Provide psychotherapy on-site Monday-Friday 8:30am-5pm or other hours when required. Balance a patient panel of 6-7 patients daily and manage a schedule of other duties. Provide short-term, structured, evidence-based psychotherapy as part of an appropriate treatment plan and in accordance with HR360 contracts. Provide clinical therapeutic services to clients, including individual and group therapy and coordination of treatment for participants with co-occurring disorders. Provide brief intervention and crisis intervention as needed, in collaboration with other departments, including de-escalation and mandatory reporting of abuse, neglect, and danger to self or others. Assessment, screening, diagnosing, arranging, and implementing client treatment plans using appropriate evidence-based tools for clients referred through the Integrated Care Center (ICC) primary care clinic. Maintain accurate and timely records of activities, progress notes, and services provided to each client. Prepare and utilize psychosocial educational materials as appropriate, needed, or required. Coordinate and collaborate with providers, case managers, and referral coordinators to improve health outcomes. Facilitate referrals to other services (e.g., case management, SUD treatment, specialty care and community resources) as needed. Work to meet client needs effectively and resolve barriers through follow-up, advocacy, and collaboration with HR360 staff and other community service providers. Participate in and/or facilitate team meetings and training. Adhere to CA Law and Ethics Standards at all times. Other duties as assigned to support the mission of the organization. Job Requirements Current possession of valid registration as an LCSW, LMFT, or Psychologist issued by the California Board of Behavioral Sciences; or if ACSW/AMFT, verified BBS Registration to accrue clinical hours. Must not be on active parole or probation. Covid-19 vaccine and booster required for this position; exemptions are not accepted.
