Final rate offered will be determined by the collective bargaining agreement, internal salary equity, candidate experience and qualifications, and budget constraints. It is not typical for an individual to be offered a rate at or near the top of the full range for a position. This position requires Illinois residency within 180 days of the hire date. Learn more: https://hospital.uillinois.edu/about-ui-health Position Summary The Transplant Nurse Coordinator (CNCII) is a Registered Nurse with experience in transplantation and living solid organ donation. Practicing under the supervision of transplant surgeons and physicians, the transplant Nurse Coordinator coordinates the clinical aspects of transplant patient and living donor care throughout the pre-transplant phase. Transplant Nurse Coordinators can be assigned to kidney, pancreas, liver and/or small bowel programs. Clinical Nurse Consultant II facilitates patient care delivery using the Nursing Process and Relationship-Based Care model, provides patient care and consultation, coordinates staff education, and models clinical practice. The CNCII aims to improve patient outcomes by integrating Shared Leadership and Evidence Based Practice. Duties & Responsibilities Evaluates patients at the appropriate UI Health site of care. Coordinates care for transplant recipients from referral and waitlisting, all potential recipients with living donors, and all living donor patients. Ensures pre-transplant patient coordination and education documentation complies with UNOS, CMS transplant program guidelines, and UI Health policies/procedures. In collaboration with the provider, orders testing for evaluation as needed and reviews results in a timely manner following UNOS and CMS guidelines. Develops, coordinates, and communicates the patient’s plan of care based on ongoing assessment and input from the patient’s family. Collaborates with the multi-disciplinary health care team to improve patient outcomes. Performs interventions as prescribed, documents accurately in the EMR, and implements orders with the provider’s guidance. Assesses and gathers clinical information to develop an appropriate patient educational plan. Recognizes symptoms, establishes priorities, and responds to changing patient needs throughout the transplant care continuum. Ensures the patient is added to the UNOS waiting list and that listing letters are sent per UNOS requirements. Coordinates and schedules living donor transplant surgeries as assigned. Participates in transplant QAPI meetings, listing meetings, department meetings, and other planning meetings. Provides cross-coverage for the transplant data coordinator by entering living donor information in UNOS prior to surgery as assigned. Engages nursing and support personnel by delegating tasks per the Illinois State Nurse Practice Act and provides support and feedback. Supports UI Health values through teamwork, patient safety, satisfaction, and a safe environment. Integrates theory, evidence-based practice, and current standards to provide quality care. Supports Shared Leadership and participates in organizational and department activities; promotes learning experiences for staff and students. Initiates, conducts, and analyzes evidence-based practice research to improve patient care outcomes; serves as preceptor, mentor, and resource to staff and students. Other duties as assigned. Minimum Qualifications High school diploma or equivalent. Graduation from an accredited school of nursing. Eligibility for registration with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation. Two years of nursing experience in the applicable field, with one year of experience at a level comparable to Clinical Nursing Consultant I. Must be able to communicate effectively in English and Spanish (written and verbal). BSN (or higher) from an accredited nursing program. Current BLS/CPR and/or other required unit-specific certifications. Current advanced cardiac life support (or within 6 months of hire). Nationally certified through ANCC or within 6 months of hire in a relevant clinical area. Preferred Qualifications Previous RN experience with solid organ transplant patients preferred.
