Independently perform home visits to conduct functional, psychosocial and environmental safety assessments. Determine eligibility of enrollees and establish an individual plan of care to enable them to remain safely at home despite long-term care needs. Use listening and communication skills with a culturally diverse population, including caregivers and other providers, to support successful care outcomes. Coordinate with interdisciplinary team members to provide the support needed without micromanagement. Regions with openings include: Bronx (Bengali or Spanish speaking preferred), Queens (Bengali, Mandarin or Cantonese speaking preferred), Brooklyn and Manhattan (Spanish, Mandarin or Cantonese speaking preferred). Additional openings in upstate counties such as Rockland, Putnam, Sullivan, Ulster, Orange, and Dutchess. Qualifications
Current NYS RN license and registration Associate degree required; Bachelor of Nursing degree preferred Bilingual BSN new graduates encouraged to apply One year of experience in Homecare, acute care or community health nursing preferred Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations Bilingual abilities in Spanish, Mandarin, Cantonese, Korean, Creole, Bengali, Hindi, and Arabic desirable Employment type
