We’re thrilled to offer a $5,000 sign-on bonus and a relocation package of $5,000 (for moves over 100 miles) or $2,500 (for moves over 50 miles) for experienced nurses joining our team!

Note: This position is specifically for nurses with 1 or more years of experience and will cover the Carle Health Hoopeston Clinic.

Hybrid Work Model: Enjoy the flexibility of working 1 day remotely and 4 days in the office!

Your Role

As an Outpatient Care Management RN, you will play a vital role in providing care management and population health services to our patients. Your focus will be on:

• Supporting patients with chronic diseases and high vulnerability during transitions between care settings.
• Coordinating medication management and care transitions.
• Creating personalized, longitudinal care plans tailored to each patient’s unique needs.
• Building effective partnerships with patients, families, and healthcare professionals.
• Utilizing data-driven approaches to enhance patient access to care and improve clinical outcomes.

What We’re Looking For

Qualifications:

• License/Certifications:
  • Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
  • Certified Case Manager within 2 years - Commission for Case Manager Certification (CCMC)
• Education: College Diploma in Nursing (Required)
• Experience: 3+ years in nursing

Your Responsibilities

• Facilitate communication and coordination across care settings.
• Identify and connect patients with appropriate providers and community services.
• Educate patients on medication adherence and self-care to minimize readmissions.
• Conduct holistic health assessments, considering health risks and social determinants.
• Document clinical and financial information accurately using appropriate systems.
• Support care coordination and chronic disease education in collaboration with the care team.

Compensation and Benefits

The compensation range for this position is $30.39 - $52.27 per hour. This range reflects our commitment to fair compensation based on experience, qualifications, and other factors.
