ALAMEDA FAMILY SERVICES

Alameda Family Services (AFS) will provide a master’s level clinical social worker who will work directly with the City of Alameda's Housing and Human Services Division (HHS) partnering with The Alameda City Library to deliver best practice social work interventions, informal mental health support to Library patrons, consultation to library staff, and linkage to services, including to City Case Managers. The clinician will provide 30 hours of on-site support per week for a term of 12 months.

(AFS) is a progressive, non-profit agency that provides clinically sophisticated, comprehensive mental health and early childhood educational and family support services to individuals and families in the City and County of Alameda. Programs include: Early Head Start, Head Start, Family Support Services, School Based Services, and Behavioral Health Care Services (Counseling and an APA- accredited Narrative Therapy Training Program). Crisis Services Division


This division encompasses clinical consultation and follow up clinical case management services for the Alameda CARE Team mobile crisis response program as well as co-located therapists and clinical case managers supporting the mental health and well-being of individuals in Alameda.


OVERALL RESPONSIBILITIES:


The Clinical Social Worker will provide trauma-informed, and culturally sensitive social work and informal counseling to library patrons. The Clinical Social Worker will conduct Social Determinants of Health (SDOH) assessments and find matching, appropriate linkages to City and County services provided to patrons who are self-referred, or library staff referred. Linkages include a warm handoff for patrons. The clinical social worker will develop relationships with hard-to-engage patrons experiencing mental health challenges. They will provide best practice clinical consultations and case conferencing, as needed to support library staff and social service providers' ability to support patrons with needs.


JOB DUTIES:

Responsibilities include, but are not limited to:


● On site therapeutic interventions, as needed with patrons

● Collaborative crisis intervention and crisis de-escalation of patrons, as requested by library staff

● Individual case management services, including resource needs assessments, case planning, referrals and warm hand offs to relevant services.

● Clinical case consultations and recommendations for engagement to library staff

● Development of relationships with patrons frequently at the library, who have chronic resource needs and mental health concerns

● Linkages to city services including Alameda City Case Managers, and other Alameda providers and county services, including to higher levels of care and specialty programs

● Case consultation to Library staff, City staff and local and county providers

● Providing training as needed and requested to library staff, in coordination with the Division Director

● Provide support and guidance to City Case Managers during warm handoffs of patrons to City staff.

● Document patron contact in EHR and other systems, according to Quality Assurance Guidelines

● Attend assigned City meetings, AFS supervision and AFS team meetings, and other meetings and trainings as assigned AFS will comply with all HIPPA regulations including obtaining a release of information prior to sharing any confidential client information. We will be limited in information sharing if the client does not agree to a release of information.


QUALIFICATIONS, SKILLS AND EXPERIENCE

● MA in Social Work

● 2 years relevant experience serving unhoused clients preferred

● Ability to work full-time fully onsite


PHYSICAL JOB DESCRIPTION

  • Ability to sit for at least 50% of the time and stand for 50%.
  • Lifting of books, manuals, and other documents weighing up to 30 pounds may be encountered in performing essential functions.


CONDITION OF EMPLOYMENT

  • A background check and clearance from the Federal Bureau of Investigation (FBI) and Department of Justice (DOJ) background prior to employment is required.
  • A Health Screening Report completed by a physician and current TB Test Results are required prior to employment.
