Director of Social Work

is responsible for evaluating and supporting themedically related social and emotional needs of the resident includingcoordination with other departments and community resources.

Qualifications: Graduate of a CSWE accredited degree program required Minimum Two-years of experience required Masters in Social Work (MSW) required Previous experience in the health care field preferred Knowledge of Microsoft Office and Microsoft Excel required Must relate well to employees, residents and visitors Must be reliable Possess exemplary follow up skills Excellent communications skills Overall professionalism Strong customer service skills Capable of working independently or as a team Creative thinking and positive attitude Responsibilities and Expectations:

Assuresadequate staffing in accordance with established budgetary constraints Assures that staff is qualified to provide quality resident care. Identifyand promote processes that will achieve employee competency Evaluates job performance of Social Worker/Social Service Designee/Coordinator, where appropriate Prepares social services schedules within budgetary guidelines Develops and maintains social services policies, procedures, objectives and standards ofpractice Responsible for assuring residents rights are met and residents, family members and staff educated on resident's rights as necessary Meets with resident/family upon admission to assess and plan to meet psychosocial needs. Provides supportive counseling as needed to adequately assist resident and family through the admission process Ensures transition of residents and their family members post-admission and during times of stress Coaches and enables other staff to respond to resident's emotional and/or psychological needs

Orients residents and families to environment and assists inadjustment through involvement in both individual and group programs Coordinates discharge planning process and arranges for resident services after discharge, including: DME; nursing care; home rehabilitation, etc Completes appropriate documentation upon admission, transfer, discharge, and completes ongoing progress notes at intervals required by regulations Completes appropriate documentation upon admission, transfer, discharge, and completes ongoing progress notes at intervals required by regulations Designs and conducts formal/informal education and/or counseling sessions for residents and family members and staff, including: intervention skills, re-motivation/ behavioral programming, orientation/coping skills and other appropriate modalities Participates in the development of the social services budget Maintains confidentiality and rights of resident, family, employee, and community information in compliance with Presbyterian Senior Living HIPAA policies and applicable regulations
