Position Summary: The Case Manager - Care Coordinator plays a vital role in supporting individuals and families experiencing homelessness and food insecurity. This position is responsible for assessing client needs, developing individualized service plans, connecting clients with resources, and providing ongoing support to help them achieve stability and self-sufficiency. In addition, this position plays a key role in the on-site food pantry operations. Key Responsibilities: Conduct assessments. Develop and implement individualized case plans, goals, benchmarks. Provide crisis intervention, advocacy, and
mental/emotional/physical
support. Connect with housing, employment, healthcare, mental health, substance abuse treatment, community resources. Maintain accurate and timely documentation. Collaborate with team, staff, volunteers, external service providers and community partners. Monitor progress and adjust service plans as needed. Facilitate life skills workshops via our Changing Lives Forever Program. Ensure compliance with organizational and departmental policies and funding requirements. Participate in team/staff meetings, trainings, and community outreach events. Provide on-Site food pantry support - deliveries, sorting, stocking, intake, ordering, inventory, Qualifications: Bachelor’s degree in Social Work, Human Services, Psychology, or related field. Minimum 2 years of experience in case management or social services, preferably with homeless or low-income populations. Knowledge of local social service systems and resources. Communication, organizational, and problem-solving skills. Ability to work independently and as part of a team. Cultural competence and sensitivity to diverse populations. Proficiency in Microsoft Office and case management software, databases. Bilingual (Spanish, etc.) a plus. Valid driver’s license and reliable transportation. Working Conditions: Fast-paced, dynamic environment. Requires lifting of supplies (25+pounds). Some travel within the community for outreach and support. Other duties as assigned.
