Overview
Clinical Specialist Pharmacist - Infectious Diseases — Full time, Day shift. Under the direction of the Clinical Pharmacy Manager, assumes responsibilities in Clinical Pharmacy with focus on Infectious Diseases and antimicrobial stewardship. Involves provision of pharmaceutical care, interdisciplinary education, problem recognition and resolution related to medication usage, and development and evaluation of medication safety programs. Utilizes just culture principles and ensures regulatory requirements for medication safety are met. Participates in pharmacist and technician clinical development programs and continuous quality improvement initiatives. Performs all pharmacist functions on a regularly assigned basis. Responsibilities
Serve as the primary Pharmacy Department liaison for Infectious Diseases issues, including committee involvement and collaboration with other Pharmacy Department staff. Partner with the Medical Director/Chair of the Antimicrobial program to develop annual priorities and metrics, with accountability to those metrics shared by the dyad. Coordinate the Antimicrobial Management Program (AMP) for the institution and other areas designated by leadership. Collaborate with Antimicrobial Management Program leads for other hospitals within the health system and Trinity Health at the state level. Mentor other pharmacists in Clinical Pharmacy and Infectious Diseases. Monitor anti-infective use in target patient populations. Identify, implement, and monitor quality and financial indicators in Infectious Diseases management for the hospital and other designated areas. Serve as clinical pharmacy liaison with Infectious Diseases physicians and provide clinical pharmacy support on Infectious Diseases rounds as needed. Publish guidelines and maintain references on the Antimicrobial Stewardship intranet site; publish annual antibiogram(s). Qualifications
Education:
Doctor of Pharmacy (Pharm.D) or clinically oriented Master’s Degree in hospital pharmacy from an accredited College of Pharmacy or equivalent are required. Experience:
Minimum of two years related experience, which may include a clinically oriented hospital pharmacy residency (e.g., PGY2). Licensure / Certification:
Current State of Michigan Pharmacist and controlled substance license or eligibility; Board Certified Pharmacotherapy Specialist in area of practice if applicable is preferred. Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification per site requirements. Our Commitment
