Provides patient/family centered nursing care via in-person and telecommunications technologies, coordinating care and supporting patients and families to manage their health care while respecting culture, values, and individual needs. Responsibilities

Provides patient/family centered care, acting as partner and adviser, assisting and supporting patients and families to manage optimally their health care, respecting their culture and values, individual needs, health goals and treatment preferences. Uses critical reasoning and clinical judgment to expedite appropriate care and treatment, including complex or potentially life-threatening conditions, often via telecommunication methods (telephone, fax, email, internet, patient portal). Ensures proper scheduling and follow-up on test results for providers; explains test results and medications to patients using approved protocols and evidence-based practice. Applies the American Nurses Association Code of Ethics to professional obligations and patient care. Participates in ongoing professional development, maintains required competencies, engages in shared governance, adheres to standards of care, and pursues lifelong learning to enhance clinical and professional responsibilities. Focuses on patient safety and quality of nursing care, identifies patient needs, conducts health education, advocates for patients, coordinates nursing and other health services, and evaluates outcomes. Maintains knowledge of care teams for coordination of patient care, self-management, population management, and effective communication. Adapts communication style to meet diverse patient, professional, cultural, and socio-economic backgrounds. Demonstrates strong assessment, critical thinking, and customer service skills. Performs other related duties incidental to the work described herein. Qualifications

Graduation from an accredited School of Nursing with two years of acute care experience required. BSN preferred. Prefer acute care experience within the last 10 years; previous experience in Telephone Triage and with Health Insurance and Managed Care concepts preferred. Current license as a Registered Nurse (RN) in the applicable state. BCLS certification required. Licensure in other states as required by law or client contract. ACLS, PALS or PEARS may be required within one year in select areas. Skills

Ability to assess nursing needs of acute and chronically ill patients through technology methods. Ability to seek resources independently and work collaboratively. Ability to establish and maintain effective working relationships. Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others. Ability to teach patients and families per the nursing plan of care; ability to use computer and learn new software programs. Ability to document and communicate pertinent information using computer and/or paper documentation tools. Work Details
