Overview
Physical Therapist (PT) – Home Health Health Link Home Health and Hospice Agency serves the greater San Francisco Bay Area and East Bay. We are looking for a compassionate, organized, and team-oriented Physical Therapist (PT) for the city of San Francisco. This is a full-time or part-time/Per Diem position. Responsibilities
Provide physical therapy services within the scope of physical therapy, in accordance with the plan of care individualized for the patient, and in coordination with the Interdisciplinary Team. Follow the plan of care; provide treatment; evaluate patient response to treatment and report any changes in condition to the Physician, Case Manager, and/or Director of Nursing. Maintain clinical records or notes and submit records according to organization policy on company devices. Provide supervision to a PTA if assigned. Adhere to all company policies and procedures. Pay
$135,000.00 - $190,000.00 annually (depending on experience) Benefits
