ChenMed is a physician-led, mission-driven primary care organization seeking to transform care for seniors. We are rapidly expanding and looking for a Primary Care Physician (PCP) to join our team, delivering affordable VIP care with a focus on outcomes and patient-centered, value-based care. The PCP will demonstrate accountability for outcomes, provide high-quality clinical care, and manage a patient panel. The role emphasizes leading a care team, patient-centered coaching, and simplifying care to drive behavior change and improved health outcomes. Essential Job Duties/Responsibilities

The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient. Take full accountability for patient care and outcomes and seek consultation from specialists when needed, while staying involved in the detailed care of the patient. Engage with the hospitalist whenever one of their patients is in the hospital, regardless of the hospitalist’s employer. Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination and documentation for patients with acute and complex chronic health needs. Lead the care team (care promoter, care facilitator, care coordinator) for patients able to come to the office. Engage with transitional care teams and other resources for patients unable to come to the office (in hospital, SNF, LTC or homebound). Active role in center management, covering for other providers as needed, and participating in recruiting patients and providers as required. Performs other duties as assigned and modified by management. Knowledge, Skills & Abilities

Availability and accessibility to build trust with patients, including walk-in visits and timely responses to calls/messages. Service orientation to provide care as one would want for a family member or themselves. Evidence-based medicine with awareness of lifestyle and nutrition factors affecting outcomes. Physician leadership and ongoing development to benefit patients, teams, centers, and the company. Commitment to high-quality care and continuous improvement in practice and center operations. Ability to influence patients and teams to achieve optimal outcomes. Self-care and wellness as essential to sustaining physician performance. Proficiency with Microsoft Office Suite and related software used by the company. Willingness and ability to travel locally (and regionally/nationally for training, recruiting, or program needs). Fluency in English (verbal and written); other languages may be required in some centers. Independent judgment in clinical settings, with appropriate collaboration as needed. Education And Experience Criteria

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required. Current State Medical License (or ability to obtain) for the state(s) of practice. Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty preferred; Board Eligibility required. Board certification maintenance through MOC, CME, or retaking exams as required. Current DEA number for schedule II-V controlled substances. Basic Life Support (BLS) certification required within first 90 days of employment.
